Clinical history

Sudden-onset right-sided weakness 2 hr · post-tPA in ER · BP 138/82 · GCS 14.

Findings

Acute infarct in the left middle cerebral artery (MCA) territory measuring approximately 1.4 cm in maximum dimension, demonstrating restricted diffusion on DWI with corresponding low ADC values. T2/FLAIR shows hyperintensity in the same region.

No haemorrhagic transformation on SWI. Mild surrounding vasogenic oedema. No significant midline shift. Ventricles and basal cisterns are preserved.

Impression

Acute left MCA territory infarct (1.4 cm), without haemorrhagic conversion. Recommend follow-up DWI at 48–72 hr and MRA correlation.
